Nursing unit conversions

Pup, the Pharm Pup mascot, thinking

Most dosage errors on exams are conversion errors. Learn these, and always convert before you calculate.

Household measures (tsp, tbsp, oz) are the values most nursing courses teach; dosing cups and syringes in mL are safer for real doses.

Big to small: multiply

Going from a bigger unit to a smaller one, multiply; from a smaller unit to a bigger one, divide.

Practice problem: 0.5 g to mg
  1. 0.5 × 1,000

= 500 mg

Practice problem: 0.1 mg to mcg
  1. 0.1 × 1,000

= 100 mcg

Practice problem: 250 mcg to mg
  1. 250 ÷ 1,000

= 0.25 mg

Practice problem: 176 lb to kg
  1. 176 ÷ 2.2

= 80 kg

Practice problem: 2 tsp to mL
  1. 2 × 5 mL

= 10 mL

Temperature

°F = °C × 1.8 + 32   and   °C = (°F − 32) ÷ 1.8

Practice problem: 38.5 °C to °F
  1. 38.5 × 1.8 = 69.3
  2. 69.3 + 32

= 101.3 °F

Writing numbers safely

  • Always use a leading zero: 0.5 mg, never .5 mg.
  • Never use a trailing zero: 5 mg, never 5.0 mg (it can be read as 50 mg).
  • Write "units" in full, never U; write mcg, not µg (ISMP).
  • Use commas for thousands: 25,000 units.

All of these are on the free dosage formula sheet.

Questions, answered.

How many mcg are in a mg?

1 mg equals 1,000 mcg. To change mg to mcg, multiply by 1,000; to change mcg to mg, divide by 1,000.

How do I convert pounds to kilograms for a dose?

Divide pounds by 2.2. For example, 44 lb ÷ 2.2 = 20 kg. Keep the decimals until the final answer.

How many mL are in a teaspoon?

1 teaspoon is 5 mL and 1 tablespoon is 15 mL, as taught in most nursing courses.
